You have spent years teaching families to pause, to ask questions, to understand their options before they choose. You have sat across from women and walked them through every path — the risks, the benefits, the implications — so they could make a decision that was truly theirs. And yet when it comes to your own practice — your own autonomy, your own future — many of us have made decisions in a moment of fear. A knock at a door. A threat from a licensing board. A colleague who got in trouble. A state that changed its laws overnight. Fear and panic are not your guides. They are your enemies. This space exists to give you what fear never could: solid ground, real information, and the clarity to make a decision that is actually yours.
There are three paths available to every midwife:
Licensing — you are permitted to serve within a defined scope, under the authority of a certifying body and your state. You have traded a measure of autonomy for a measure of protection. But do you know exactly what you traded? And what it costs the families you serve?
LLC — you have a business structure that offers some liability protection. But do you know what that structure does to the private nature of your relationship with the families you serve? And what it signals to the state about who you are and how you operate?
Unlicensed private service — you are operating outside the public regulatory system, grounded in your constitutional rights (US), the UDHR, or the Canadian Charter of Rights. There is no path that removes all risk. But there is a path built on solid ground, real knowledge, and clear eyes. Most midwives have never had all three of these options laid out honestly in front of them. Most midwives made their choice — whatever it was — with incomplete information, under pressure, or in a moment of fear. This is where that changes.
When fear is driving, we make decisions that protect us from the feeling — not decisions that align with our calling. We get licensed because we are afraid of what happens if we don't — not because we understand what licensing actually means for our autonomy and the families we serve.
We form an LLC because someone told us it would protect us — not because we understand what that structure actually communicates to a state that is looking for a reason to regulate us.
We walk away from midwifery altogether because we cannot find solid ground — not because we were never called.
